What To Expect On Your First Day of Physical Therapy

Please arrive 15 minutes early to fill out the necessary paperwork if you have not already completed it online, as well as any medical records in relation to why you're being seen at our clinic. Prepare to spend at least an hour with your physical therapist so they are able to properly assess your condition and create the appropriate treatment program for you. Wear comfortable clothing that allows easy access to your area of treatment.

Do you accept my insurance?

Makovicka Physical Therapy accepts all major insurance plans. We are recognized as a Tier I/Select provider for all Blue Cross Blue Shield policy holders. If you have a question about your insurance coverage, please contact any of our eight locations, and we'd be happy to help.
